Each team will be supplied with patches for two uniforms per player at no cost to the team. There is a limited number of additional patches that can be purchased for $1 each.
*NOTE: Currently, purchasing extra patches on site at the first National League events is the only option. Patches are no longer available to ship to teams ahead of the events.
Additional uniform patches can be ordered in one of two ways:
1. MAIL-IN ORDER; PATCHES SHIPPED TO TEAM - Mail a check and this form to US Youth Soccer. Shipping charge will apply.
After sending the form and payment to US Youth Soccer, your patches will be shipped to the mailing address you provide on the form. If ordering patches for multiple teams that will be sent to the same address, submit a single form/order to save on shipping costs.
2. PURCHASE AT FIRST EVENT - Teams will be able to purchase patches (pending availability) beginning on the first day of play at the site of their respective first event of the season. In order to obtain patches, you must provide a check at the time of the pick up.
On-site pickup is subject to availability. While on-site pickup will eliminate shipping fees, there is a smaller chance that extra patches will be available on site due to prior mail-in orders.
